Zusammenfassung: Oral and maxillo facial prosthesis aims an anatomical, functional and aesthetic rehabilitation through non-living substitutes in regions of the maxilla, mandible and face that are absent or defective. It’s necessary in losses of support skeleton, muscle and lining tissue, as it promotes the protection of the remaining structures exposed and it is important in the rehabilitation and reinsertion in the social life of patients who suffering from acquired or congenital deformities. Their functions are among the reestablishment of speech, chewing, swallowing, breathing, as well as aesthetics, attenuating the deformity. The aim of this study was to present a clinical case of rehabilitation by oral and maxilo facial prosthesis in the palate and eyelid oculus region after oncologic surgery. Patient ARC, male, 62 years old was referred to the Special Patients Dental Care Center (Centro de Especialidades Odontológicas da Universidade do Estado do Amazonas – CEO/UEA), after a mutilating surgical treatment of maxillary by sinus lesion whose diagnosis was low-differentiated and extensively necrotic carcinoma at the Oncology Control Center Foundation of the State of Amazonas (Fundação Centro de Controle de Oncologia do Estado do Amazonas – FCECON). Treatment consisted in total left maxilectomy, removal of parts of the zygomatic bone, exenteration of the eyeball, and parts of the frontal bone. As complement of the treatment the patient also did radiotherapy and chemotherapy. After evaluation of the patient it was verified the possibility of using an oral and maxillo facial palatal obturator intraoral and facial eyelid oculus prosthesis. The prostheses were made by thermopolymerized acrylic resin and silicone, and were installed, softening the suffering of the patient. Dental surgeon has an important role in the rehabilitation of patients with congenital or acquired deformities, which can maintain or improve their quality of life and help their social reintegration.
Beschreibung: A prótese buco maxilofacial tem como objetivo a reabilitação anatômica, funcional e estética através de substitutos não vivos em regiões da maxila, mandíbula e face, que estejam ausentes ou defeituosas. Faz-se necessária em perdas do esqueleto de suporte, tecido muscular e de revestimento, pois promove a proteção das estruturas remanescentes expostas e é importante na reabilitação e reinserção no convívio social de pacientes que sofrem com deformidades adquiridas ou congênitas. Entre suas funções estão o restabelecimento da fala, mastigação, deglutição, respiração, assim como a estética, atenuando a deformidade. O objetivo deste trabalho foi apresentar um caso clínico de reabilitação por prótese buco maxilofacial em palato e região óculo-palpebral após cirurgia oncológica. Paciente ARC do gênero masculino, 62 anos, foi encaminhado ao Centro de Especialidades Odontológicas da Universidade do Estado do Amazonas – CEO da UEA, após sofrer um tratamento cirúrgico mutilador por lesão em seio maxilar cujo diagnóstico foi carcinoma pouco diferenciado extensamente necrótico na Fundação Centro de Controle de Oncologia do Estado do Amazonas – FCECON. Seu tratamento consistiu em maxilectomia total esquerda, remoção de partes do osso zigomático, exenteração do globo ocular e parte do osso frontal. Como complemento do tratamento também fez sessões de radioterapia e quimioterapia. Após avaliação do paciente constatou-se a possibilidade do uso de próteses buco maxilofaciais intrabucal obturadora palatina e facial óculo-palpebral. As próteses foram confeccionadas em resina acrílica termopolimerizável e silicone e instaladas, amenizando o sofrimento do paciente. O cirurgião- dentista tem um papel importante na reabilitação de pacientes com deformidades congênitas ou adquiridas; podendo manter ou melhorar sua qualidade de vida e ajudar em sua reinserção social.
