dc.description.abstractABSTRACT Iron deficiency (ID) is a health condition in which the availability of iron is insufficient to meet the needs of the body. This condition remains the most common cause of anemia, accounting for half of all cases of anemia worldwide, prevailing even in developed countries. Clinically, it manifests itself through generalized fatigue, pica, hair loss, dysphagia, brittle nails, among others. Recent data show that the frequency of ID is high in blood donors, and that this situation is even more complex in women at reproductive ages due to losses related to menstruation, pregnancy and lactation. However, a high proportion of blood donors are deficient in iron despite sufficient hemoglobin levels for donation. In this study, we evaluated the accuracy of two clinical instruments (CI), based respectively on anamnesis and physical examination (PE), with the objective of detecting iron deficiency without anemia among female blood donors who attended blood collection unit of a public blood center in Brazil (HEMOAM),thus providing data for an improvement in standard screening quality for donation. The study was cross-sectional, descriptive and analytical. The study population consisted of 221 approved blood donors in the blood donation screening process at the HEMOAM central collection unit. The clinical data were obtained by means of an instrument for anamnesis and PE. The anamnesis CI tool consisted of three questions, focused on: (1) pica symptoms; (2) hair loss; and (3) nail changes, and the CI of PE consisted of a standardized physical examination, we evaluated (1) pallor of the palpebral conjunctiva, and (2) nail weakness. Determinations of serum ferritin, transferrin saturation, serum iron and hemogram were performed. The accuracy of the CI was measured by sensitivity, specificity, predictive values, and likelihood ratios. The mean age of the population was 31 years. The frequency of blood donors with ID was 40% (ferritin <30 ng/ ml), and 9.9% of the donors had a more severe deficiency (ferritin <30 ng/ml).Regarding the risk factors for ID, significant, yet a mild correlation (p<0.05) was observed between ferritin and duration of menstruation (P=0.0037).Regarding the use of the CI, none of the analyzes of the clinical criteria used for the diagnosis of ID presented individually acceptable values of accuracy. By analyzing the three questions in the clinical history together and using the affirmative criterion for only one of them, we noticed a clinically significant increase in the sensitivity of this criterion in the different cutoff points of ferritin and hemoglobin (Hb),as well as higher values of predictive value negative (77%, 94% respectively with ferritin <12ng/ml).The rationale for this strategy was to try to create a panel of questions that applied to the screening, xi could be used to exclude donors with higher risk of ID, although some of these results were false positives. Finally, when considered together all the variables that indicate ID, from the clinical point of view presented higher values of sensitivity and negative predictive value, indicating the possibility of using this clinical criterion as a tool for clinical screening for ID among females blood donors, particularly in areas with higher ID prevalence. dc.description.resumoRESUMO A deficiência de ferro (DF) é uma condição de saúde em que a disponibilidade de ferro é insuficiente para atender às necessidades do corpo. Essa condição continua sendo a causa mais comum de anemia, alcançando a metade dos casos de anemia em todo mundo, prevalecendo mesmo em países desenvolvidos. Clinicamente, ela se manifesta por fadiga generalizada, pica, perda de cabelo, disfagia, unhas frágeis, entre outras. Dados recentes mostram que a frequência de D Fé alta em doadores de sangue, e que esta situação é ainda mais complexa em mulheres em idades reprodutivas, devido às perdas relacionadas à menstruação, gravidez e lactação. No entanto, umas elevadas proporções de doadoras de sangue são deficientes em ferro, a pesar dos níveis de hemoglobina suficientes para doação. Neste trabalho de pesquisa avaliamos a acurácia de dois instrumentos clínicos (IC), baseados respectivamente na anamnese e no exame físico (EF), com o objetivo de detectar deficiência de ferro sem anemia em doadoras de sangue que compareceram na unidade de coleta de sangue da Fundação HEMOAM, assim fornecer dados para uma melhoria na qualidade de triagem padrão para doação. O estudo realizado foi de corte transversal, descritivo e analítico. A população estudada foi constituída de 221 doadoras de sangue aprovadas no processo de triagem para doação de sangue na unidade de coleta central de HEMOAM. Os dados clínicos foram obtidos por meio de um instrumento para realização de anamnese e do EF. O IC de anamnese testado consistiu em três perguntas, focadas em: (1) sintomas de pica; (2) alterações capilares; e (3) alterações ungueais, e o IC de EF consistiu em um exame físico padronizado, foram avaliados: (1) palidez da conjuntiva palpebral; e (2) fragilidade das unhas. Foram realizados as determinações de ferritina sérica, saturação de transferrina, ferro sérico e hemograma. Já a acurácia dos IC foi mensurada por meio da sensibilidade, especificidade, valores preditivos, razoes de verossimilhança. A idade média da população foi de 31 anos. A freqüência de doadoras de sangue que apresentavam DF foi de 40% (ferritina <30 ng/ml), e 9,9% das doadoras apresntavam uma deficiência mais grave (ferritina <30 ng/ml). Quanto aos fatores de risco para DF, uma correlação significativa, mas ligeira (p <0,05) foi observada entre a ferritina e a duração da menstruação (P = 0, 0037). No que diz respeito ao uso do IC, nenhuma das análises dos critérios clínicos utilizados para diagnostico de DF apresentavam individualmente valores de acurácia aceitáveis. Analisando-se conjuntamente as três perguntas da história clínica, e usando como critério de positividade a resposta afirmativa para apenas uma delas, notamos um aumento clinicamente significativo da ix sensibilidade deste critério nos diferentes pontos de corte de ferritina e hemoglobina (Hb), assim como valores mais elevados de valor preditivo negativo (77%, 94% respectivamente com ferritina < 12 ng/ml). O racional desta estratégia foi tentar criar um painel de perguntas que aplicado à triagem, poderia ser usado para a exclusão de doadores com maior risco de DF, ainda que parte destes resultados fossem falsos positivos. Por fim, quando considerado em conjunto as variáveis que indicam DF, do ponto de vista clinico apresentaram maiores valores de sensibilidade e de valor preditivo negativo, indicando a possibilidade de utilização deste critério clínico como instrumento de triagem clínica para DF entre doadoras de sangue, particularmente em áreas com maior prevalência de DF.
